10公里健康跑免责协议及中方人员报名信息登记表
一、免责协议
免责协议：
参加2020年2月8日万通银行杯2020拉各斯城市马拉松10公里健康跑活动，对身体状况不佳的选手，可能构成健康威胁。参与活动者本人需阅读此免责协议并签字证明本人对本次活动所涉及的风险有充分的了解，且本人身体健康，接受过充分的培训，能够参加本次活动，本人同意不伪造使用身份，不使用用于宣传和广告目的的照片和录像带。活动参与过程中出现任何财物丢失、受伤、死亡等意外情况，皆与包括尼日利亚中国总商会在内的活动组织单位、组委会和所有赞助商、合作伙伴、机构、子公司无关，以上公司、单位不承担任何责任，免除任何诉讼、索赔。
凡成功报名参赛者，即认定为知悉并同意以上免责协议。

WAIVER AND RELEASE FOR LIABILITY
WARNING: Participation in the Access Bank Lagos City Marathon can be a serious threat to the health of individuals who are not in excellent physical condition. For and the consideration of my participation in the Access Bank Lagos City Marathon, I myself, my executors, administration, heirs and assigners hereby release and discharge the Athletics Federation of Nigeria (AFN) , Nilayo Sports management limited, the organizing committee and all sponsors, partners, agencies, subsidiaries, affiliates and beneficiaries jointly and severally and hold and waiver harmless from and against any and all action, claims, injuries incapacitations /death demands, liabilities, loss, damage and expenses of whatever kind and nature including, but not limited to, attorney fees which at anytime may be incurred by reason of my participation in or my preparation for the said event.
I attest and verify that I have full knowledge of the risk involved in this event and I am physically fit and sufficiently trained to participate. The undersigned grants full permission to any and all forging use to his/her likeness, including photographs and video tapes for publicity and advertising purpose without compensation.
Every runner to present a medical certificate stating that THERE IS NO CONTRADICTION TO PARTICIPATION.
